Unlock Your Life™ Review Agreement

1. Overview

The Unlock Your Life™ Review is a personalised insight session designed to help you interpret your chosen Precision Diagnostic in one of four key areas of life: Health, Wealth, Relationships or Meaning.

Your Review includes:

• A personalised Unlock Your Life™ assessment link for one chosen area of life.

• A 15-minute online Precision Diagnostic covering 10 key metrics.

• A comprehensive Diagnostic Report highlighting your strengths, growth opportunities, patterns, blind spots and priority areas.

• One 60-minute private Unlock Your Life™ Review with Paula Smith.

• A personalised interpretation of your results.

• Exploration of the three priority areas identified within your assessment.

• Insight into the patterns influencing your current experience.

• Thought-provoking questions to deepen your awareness and understanding.

• Personalised recommendations and practical next steps.

• A copy of your personalised Diagnostic Report following your Review.

2. My Commitment to You

As your facilitator, I will provide a supportive, confidential and non-judgemental environment in which to explore your assessment.

I will bring:

• My full presence and attention.

• Professional coaching experience and expertise.

• Honest observations and thoughtful questioning.

• Respect, integrity and confidentiality.

3. Your Commitment

To gain the greatest value from your Review, you agree to:

• Complete your Precision Diagnostic at least 48 hours before your appointment.

• Attend your scheduled Review on time.

• Participate openly and honestly.

• Take responsibility for your own decisions and any actions you choose to take following the session.

4. Session Details

• Your Review lasts approximately 60 minutes.

• Sessions are held online via Zoom.

• A Zoom link will be provided after booking.

• Sessions are not routinely recorded.

5. Rescheduling & Cancellation

If you need to rearrange your appointment, please provide at least 24 hours' notice.

Appointments cancelled with less than 24 hours' notice, or missed without notice, may be forfeited and are non-refundable except in exceptional circumstances.

6. Confidentiality

Everything discussed during your Review will remain strictly confidential.

The only exception is where I believe there is a legal or safeguarding obligation to disclose information due to a serious risk of harm to yourself or another person.

7. Scope of the Review

The Unlock Your Life™ Review is designed to provide insight, interpretation and personal development guidance based on your Precision Diagnostic.

It is not therapy, counselling, medical advice, financial advice or legal advice.

You remain responsible for your own decisions and actions.

8. Intellectual Property

Your Diagnostic Report and all accompanying materials remain the intellectual property of Paula Smith Coaching and are provided for your personal use only. They may not be reproduced, distributed or shared without written permission.

9. Client Acknowledgement

By purchasing the Unlock Your Life™ Review, you confirm that you have read and understood this agreement and agree to participate openly and honestly in the process.
